Community-Based Prevention Services

Our Community-Based Prevention Services case managers work with Lowcountry families to build strong and lasting futures by providing access to community resources, early intervention referrals, and in home services.

This is accomplished by:

  • Family Assessments to identify safety and risk factors and development of a care plan to address findings.
  • In home interventions that teach techniques to decrease life stressors.
  • Parental training to help families provide a safe, nurturing home for their children.
  • Connections to community resources for self-sufficiency, housing options, medical care, and educational/ vocational opportunities.
  • Care plan progress monitoring to support families in achieving their goals.

Dreams Realized Pam

Fleeing an abusive marriage, Pam, a single mom of 4, came to South Carolina to live with her brother. When her brother received an eviction notice, she and her children became homeless. Her youngest boy began having accidents on himself at school and trauma was the suspected cause. With guidance from a Windwood case manager, Pam found mental health services for her son, employment for herself and housing to keep the family together.

Dreams Realized Wanda

A DSS referral, Wanda, a disabled Veteran, had black mold in the family home where she lived with her two girls and father to save funds and return to college. After a needs assessment, case managers provided Wanda with referrals to housing providers, a community resource guide, canned goods, encouragement, and praise for each positive step. In July, Wanda received word from the VA that she received transitional housing and now has a new home.

Dreams Realized Miranda

Miranda and her three autistic children lived in a home with a man that had become abusive. He refused to help with household expenses. When Windwood staff met Miranda she was scared, felt trapped in her own home, and struggled financially. She and her boys spent most of their time hiding in their bedrooms to avoid abuse.

With our help, Miranda developed a plan to find employment and a new home away from the abuser. Despite encountering challenges along the way, Miranda found a steady job, a new apartment, bought a car, and started a savings account. She and her boys are finally living a peaceful, stable life.

We are pleased to provide these referral-based services in the tri-county area under the direction of our master contractor, SAFY.
